What do I need on my first visit?
Please bring the completed patient information form
and your insurance card or current medical coupons.
Do I need to bring copies of my medical records?
Your doctor will send them to the
Pain Center prior to your appointment. This allows
an extensive record review before you visit, and
ensures that you will receive the best care
possible. If there are X-rays or an MRI available,
you may be asked to bring them to your appointment
to ensure their safe arrival.
When should I arrive for my appointment?
Plan to arrive at least 30 minutes before your
appointment to check in with the front desk to
complete necessary paperwork, where the staff will
prepare your medical chart and process your
insurance forms.
Do I need a referral from my primary doctor?
Please contact your insurance company for this
information.
Does SERPC accept Georgia Worker's Comp?
Yes.
Does SERPC accept Medicare? GA. Medicaid?
